In 2006, the Joint Commission
reported that the majority of medical errors occur during the transitions of
care due to the lack of effective communication amongst a patient’s care team.
They reported that a primary driver of ineffective communication is information
that is lost or inaccessible during the care process. As the adoption of
clinical systems in acute care and ambulatory environments has increased, the
